1.All information collected from this Questionnaire will be kept strictly confidential, and for the purpose of internal evaluation by HR and Admin department only;
2.Amendment is not allowed after your submission. If you there is any change to the information submitted (e.g. health status or location), please delete your previous submission and provide a new response；
3.Please self-quarantine for 14 days if you return from or were in transition at the Hubei Province, or have been in contact with persons who are from the Hubei province.
4.Except Hong Kong, overseas colleagues do not need to fill in.
